I have not customized the product other than themes as well as using an existing membership provider integrated with another app. YAF is running under an existing site.

I see another user having issues with watch emails and am not sure if it is related. I can send direct, pm, emails without issue so email settings should be setup correctly.

What's the process to troubleshoot email sending?

Next steps?

Why would some emails work but not others?

I am having the same issue as well. Same setup, except using IIS 7. The e-mails go out for forum signup, but no notifications are going out for moderation messages.

I had a few other things going on that stopped emails form being sent. I downloaded some updated DLL's from another thread and started to get error messages in the admin log that said I had some domain rejection issues from my SMTP server.
